Complex Home Modification Assessment (CHM)
At Ability OT we provide Complex Home Modifications Assessments (CHM). These assessments help determine the scope and complexity of client's conditions and provide modifications to their home to improve their living conditions and quality of life.
Eligible clients may have access to funding from the NDIA.
Complex Home Modifications
- Highly Complex Works that Impact Multiple Areas of the Home
- Higher Risk of Problems Occuring Throughout Process
- Particpant Unable to Use Areas of the Home
- Cost More Than $10,000
- Requires Building Approvals or Permissions
- Requires Certification of Works (i.e. Plumbing, Electrical Works, Landscaping, Carpenter)

Lifts (Platform and Vertical)
Stepless Shower
Minor Home Modifcations
- Non-structural
- Low Risk of Major Problems
- Affect Minimal Areas of the Home (One or Two)
- Cost Less Than $20,000

Simpe Ramps
Temporary Rams
Grab Rails
Moving or Adding Taps
Removing or Changing Internal Doors or Shower Screens
Process for Complex Modifications - NDIS Participants
1. Assessment of the home environment
2. Ocupational Therapist draws up concept design and basic scope of work needed
3. Joint visit by the ocupational therapist and chosen tradespeople
4. Builder and other tradespeople to complete scaled drawings and provide itemised quote for work
5. Ocupational therapist to complete NDIS application paperwork
6. Ocupational therapist to confirm designm scope anf application with participant and submit to NDIS
7. Await NDIS response
8. NDIS response. If successful builder notified. COntract to be signed between participant and builder. Work to commence
9. Ocupational therapist to complete progrss visits as required (compile defects list, if any)
10. Ocupational therapist final visit to ensure participant satisfaction and needs are met
